About the Role Our established Community Services team is seeking a dedicated Care Manager. You will provide personalised support to clients living at home, ensuring their care aligns with their individual needs and goals. Reporting to the Manager of Community Services you will: - Lead quality service delivery across your client group
- Manage Support at Home budgets effectively and responsibly
- Maintain clear and efficient communication with clients, families, staff, and external stakeholders
- Ensure compliance with all regulatory and accreditation requirements
- Deliver services in line with Client Agreements and organisational standards
- Work collaboratively with your team to provide integrated, holistic care
- Maintain up‑to‑date knowledge and skills relevant to the role
- Promote a positive workplace culture and uphold Royal Freemasons’ Core Values
- Complete internal audits and reporting within required timeframes
- Operate in a hybrid environment — working from home, visiting clients, and occasional office attendance
Skills & Experience We are looking for candidates who bring: - A relevant qualification in Community Services, Health, or Management (essential)
- A current Victorian Driver’s Licence and reliable, comprehensively insured vehicle (essential)
- Experience with Support at Home (highly regarded)
- Knowledge of Aged Care Act 2024, Strengthened Aged Care Quality Standards, and community care practices
- Competence with MS Office and relevant information systems
- Understanding of Consumer Directed Care principles
- Strong budgeting, negotiation, and communication skills
- A genuine commitment to supporting older people
- A current NDIS Worker Screening Check and valid Australian working rights
